As per the County of Wellington's Administrative Monetary Penalty By-law, registered owners that have received a Penalty Order under the ASE Programme may request a review of their subject Penalty Order by a Hearing Officer within 30 days of receipt of their Notice of Decision from the Screening Officer.
If you, as the registered owner of the vehicle, have lost your Penalty Order, or would like to obtain another copy of your Penalty Order and/or the Certified Statement of the Provincial Offences Officer prior to your Hearing, you are required to complete and submit this form, at minimum, seven days prior to your scheduled Hearing. Documents will not be provided until such time as confirmation of your scheduled booking request has been received.
